Job Title: Registered Nurse (RN) Case Manager
Location: Hyannis, MA
Assignment Duration: 13 Weeks
Schedule: 4x8 Day Shifts
Weekly Gross Pay: ~$2,296
We are seeking an experienced RN Case Manager to join the care coordination team in an acute care hospital environment. This role involves discharge planning, utilization review, and working collaboratively with interdisciplinary teams to ensure safe, efficient, and patient-centered care transitions.
Bachelor of Science in Nursing (BSN) preferred
Minimum 3 years recent acute care experience within the past 5 years in a hospital setting or case management.
Working knowledge of InterQual® or an equivalent utilization review system.
Recent experience (within past 4 years) in Discharge Planning and Utilization Review required.
Certification in Case Management (CCM, ACM, CPUM, etc.) preferred.
Strong interpersonal and negotiation skills; able to maintain professional interactions with patients, providers, payers, and staff.
Strong organizational and time management abilities with the capability to prioritize effectively.
Ability to work independently in a fast-paced and sometimes high-pressure environment.
Proficient computer and documentation skills.
Screen new admissions to identify discharge planning needs.
Perform initial and ongoing patient assessments in InterQual® based on clinical status.
Review appropriateness of hospital admissions and level of care.
Coordinate discharge planning and secure post-acute services when indicated.
Provide education and communicate with patients and families regarding care transitions.
Conduct utilization reviews and respond to payer requests for ongoing clinical information.
Collaborate with physicians, nursing, therapy teams, and external agencies to support care continuity.
Facilitate patient appeal rights and issue required notices when applicable.
Act as patient advocate and resource for community-based support services.
Participate in interdisciplinary rounds and care planning discussions.
Identify and address barriers to discharge.
Assist in coding and reimbursement processes by ensuring complete documentation.
Maintain compliance with regulatory, safety, and infection control standards.
Participate in department meetings, education programs, and continuous improvement initiatives.
Perform other related duties as assigned in support of patient care delivery.
Active Registered Nurse (RN) License in the state of Massachusetts
BLS (AHA) certification required
