Weekly Gross Pay: $2,374
We are seeking an experienced Registered Nurse – Case Manager for a 13-week day shift assignment in Syracuse, New York. The ideal candidate will have a strong background in case management and utilization review within an acute care hospital environment, ensuring optimal patient outcomes and efficient care coordination.
Coordinate and manage patient care from admission through discharge, ensuring appropriate resource utilization.
Perform clinical reviews and ensure compliance with hospital and payer policies.
Collaborate with interdisciplinary teams to develop and implement individualized care plans.
Facilitate transitions of care and discharge planning in collaboration with patients, families, and providers.
Document case management activities accurately within the EMR system.
Support utilization management and clinical documentation improvement (CDI) initiatives.
License: Active and unrestricted Registered Nurse (RN) license in the State of New York.
Certifications: BLS (required).
Experience:
Minimum 1.5 years of recent acute care hospital experience (within the last 2 years).
Case Management and Utilization Review/CDI experience required.
Prior experience in a hospital of similar size or a trauma center preferred.
Technical Skills: Proficiency in EPIC Electronic Medical Records (EMR).
Strong communication, analytical, and critical thinking skills.
