Position Overview: The Manager of Social Services provides leadership for hospital-based social work programs, overseeing discharge planning operations to ensure safe, timely patient transitions and optimal continuity of care.
Key Responsibilities:
• Lead, supervise, and develop the social work team and daily departmental operations.
• Collaborate closely with case management, nursing, physicians, and interdisciplinary teams.
• Ensure adherence to regulatory standards and protection of patient rights.
• Oversee discharge planning workflows and coordination of internal and community resources.
• Advance process improvement, patient satisfaction, and care efficiency initiatives.
Qualifications:
• Active LCSW or equivalent licensure.
• Minimum 1 year of leadership experience in hospital social work or case management.
• Previous acute care hospital experience required.
