Pay Range(Local): $54–$58/hour ($2,295 – $2,460 weekly)
Overview:
"Navitas Healthcare, LLC" is seeking Case Management RN for an exciting job in Hyannis, MA.
Responsibilities:
Assess and screen new admissions to determine discharge planning needs.
Complete initial and ongoing patient assessments using InterQual® or similar criteria.
Conduct utilization reviews and collaborate with payers to support reimbursement.
Coordinate interdisciplinary care planning and complex discharge needs.
Communicate care options, denials, and coverage details to patients and families.
Facilitate post-acute transitions with community providers or home care services.
Support quality initiatives, care pathway review, and documentation accuracy.
Maintain compliance with hospital policies, safety, and infection control standards.
Qualifications:
Active RN license in Massachusetts.
3+ years recent acute care experience; discharge planning/utilization review strongly preferred.
Experience with InterQual® or similar review tools.
BSN and Case Management certification (CCM / CPUM) preferred.
Strong communication, critical thinking, and coordination skills.
Proficient in EMR and clinical software systems.
