Job Title: Travel RN – Case Manager
Location: Fort Myers, FL 33912
Duration: 13 Weeks
Shift: 8-Hour Day Shift (Monday–Friday, 8:00 AM – 4:30 PM)
Guaranteed Hours: 40 hours per week
Pay Rate: $2,160 (Weekly Gross)
Radius Rule: Must reside at least 50 miles from the facility
We are seeking an experienced Travel RN Case Manager to join a leading healthcare team in Fort Myers, FL. The ideal candidate will play a key role in coordinating patient care, ensuring effective discharge planning, and promoting high-quality outcomes across the continuum of care. This position requires strong communication skills, clinical judgment, and a proactive approach to managing complex cases in an acute care environment.
Assess, plan, coordinate, implement, and evaluate patient care needs throughout hospitalization.
Collaborate with physicians, nurses, and interdisciplinary teams to ensure safe and efficient discharge planning.
Facilitate transitions of care by arranging home health, rehabilitation, or long-term care services as needed.
Review patient charts to ensure appropriate utilization of resources and adherence to clinical pathways.
Advocate for patients and families to promote informed decision-making and continuity of care.
Monitor and document patient progress, barriers to discharge, and care outcomes.
Comply with all hospital policies, accreditation standards, and regulatory requirements.
Provide education to patients and families regarding care plans, post-discharge instructions, and available resources.
Serve as a liaison between patients, families, and healthcare professionals to ensure coordinated and compassionate care.
Float to other departments or affiliated campuses as needed.
Required:
Active Registered Nurse (RN) license in Florida or Compact state.
Minimum 2 years of recent Case Management experience in an acute care hospital setting.
Current BLS (Basic Life Support) certification.
Strong knowledge of discharge planning, utilization review, and care coordination.
Preferred:
Bachelor’s degree in Nursing (BSN) or higher.
Familiarity with hospital EMR systems.
Excellent communication and organizational skills.
