Hours per Week: 37.5
Weekly Pay Range: Up to $2,490
"Navitas Healthcare, LLC" is seeking Licensed Clinical Social Worker for an exciting job in Hyannis, MA.
Active Massachusetts LCSW License (Required)
Crisis Prevention Intervention (CPI) Certification
Basic Life Support (BLS) Certification
Excellent communication and clinical documentation skills
Experience in behavioral health, psychiatric, or acute care settings preferred
Strong interpersonal, organizational, and crisis management skills
Conduct comprehensive clinical assessments for patients with behavioral health needs.
Develop and communicate individualized treatment and discharge plans.
Provide therapy and counseling services, including individual, group, and family sessions.
Collaborate with psychiatrists, nurses, and other healthcare professionals to ensure quality care.
Coordinate with community agencies and services to support patients’ post-discharge needs.
Assist patients and families with planning, resources, and placement for continued care.
Provide crisis intervention and prevention support as required.
Maintain compliance with state and federal regulations and documentation standards.
Participate in care coordination meetings and community outreach programs.
Deliver compassionate, patient-centered care and maintain professional relationships.
Master’s Degree in Social Work from an accredited institution (required)
