Job Title: Physical Therapist
Location: Cortlandt Manor, NY
Assignment : 13 weeks
Shift: 12:00 PM – 8:30 PM (30-min break included) | Rotating shifts between 8:00 AM – 8:30 PM as needed
Department: Physical Therapy
We are seeking a dedicated and experienced Physical Therapist for a 13-week assignment to provide coverage for a full-time leave of absence. The ideal candidate will have strong outpatient experience, familiarity with EPIC, and a proven ability to evaluate, plan, and administer treatment programs aimed at restoring function and preventing disability after illness or injury.
Perform patient evaluations and develop individualized treatment plans.
Direct, administer, and document physical therapy treatments in accordance with professional standards.
Collaborate with interdisciplinary healthcare teams to ensure coordinated patient care.
Educate patients and families on therapy plans, home exercises, and preventive care.
Monitor progress and adjust treatment plans as needed.
Maintain accurate medical records and comply with departmental policies.
Contribute to maintaining a safe, supportive, and efficient clinical environment.
Active New York State Physical Therapy License (required).
CPR Certification (preferred).
Outpatient care experience (highly preferred).
EPIC EMR experience (highly preferred).
Ability to work flexible rotating shifts between days and evenings.
