Job Opportunity: Licensed Practical Nurse (LPN) – Skilled Nursing Facility 

Location: Cooperstown, NY 

Duration: 13-Week Contract 

Position Overview 

We are seeking a dedicated Licensed Practical Nurse (LPN) for a 13-week assignment in a skilled nursing facility. The LPN will play a key role in delivering quality care to residents, fostering a safe and supportive environment, and collaborating closely with the healthcare team to promote resident wellness. 

Key Responsibilities 

• Provide hands-on nursing care, administer medications, and monitor residents’ health status 

• Assist residents with activities of daily living, ensuring comfort and dignity 

• Observe and report changes in condition to RNs or providers promptly 

• Maintain accurate documentation and comply with regulatory standards 

• Participate in individualized care planning and goal setting 

• Support residents’ emotional needs with compassion and respect 

• Collaborate with families and interdisciplinary team members for holistic care 

• Uphold infection prevention, safety guidelines, and facility protocols 

• Guide and oversee CNAs and other support staff in daily care delivery 

Requirements 

• Valid LPN license in New York State 

• Graduate of an accredited Practical Nursing program 

• Active BLS certification 

• Prior experience in LTC, skilled nursing, or geriatric care preferred 

• Strong communication, organizational, and team skills
