Job Title: Licensed Practical Nurse (LPN)

Duration: 1 year contract (Extendable)

Work Location: Flagstaff, AZ

Shift Timing: 12-hour shifts, 6:00 AM – 6:00 PM

Requirement: AZ RN license, and BLS

Job Summary: Navitas Healthcare, LLC is seeking LPN for  an exciting job in Flagstaff, AZ.

Job Description:

• Observe and report the effectiveness of medications and treatments
• Ensure adequate supplies and medications are available for the unit
• Deliver and receive information regarding residents’ medical, social, and functional needs
• Participate in shift change reports to ensure continuity of care
• Communicate with residents, families, and staff to support resident-centered care
• Conduct initial admission nursing assessments for new residents
• Assist in completing MDS assessments, quarterly reviews, and developing care plans
• Maintain accurate clinical and administrative documentation on resident conditions, treatments, and progress
• Attend and actively participate in staff meetings
• Principles of gerontological/geriatric nursing and aging processes

Qualifications:

• Current and active LPN license (state-specific)
• Previous experience in long-term care or geriatric nursing preferred
• Ability to work 12-hour shifts and flexible weekly hours (38–48 hours/week)
