Pay Rate: $1,400 to $1,612.50 per week

Join our Long-Term Care (LTC) team, where you'll play a vital role in delivering high-quality care to residents in a supportive, team-oriented environment. You will work under the supervision of an RN or clinical supervisor, providing hands-on care, administering medications, monitoring resident health, and ensuring comfort and safety.
Provide direct nursing care to residents in accordance with care plans and physician orders
Monitor resident health and report changes in condition to RN or physician
Administer medications and treatments as prescribed
Assist with assessments and contribute to care planning and documentation
Supervise and support CNAs or other care staff
Maintain accurate and timely documentation in medical records
Ensure compliance with facility policies, state, and federal regulations
Promote a safe, clean, and supportive environment for residents
Educate residents and families about care procedures and health conditions
Participate in shift handoffs, meetings, and continuing education as required
Graduate of an accredited LPN/LVN program
Current and valid LPN license in the state of practice
CPR/BLS certification (required)
Previous LTC or geriatric nursing experience (preferred)
