The Licensed Master Social Worker (LMSW) will provide comprehensive psychosocial services to patients in inpatient medical/surgical units, supporting care transitions, crisis management, and discharge planning.
Conduct psychosocial assessments for patients admitted to inpatient med/surg units
Develop and implement individualized care plans in coordination with physicians, nurses, and allied health staff
Provide crisis intervention, grief counseling, and emotional support during acute illness, trauma, or hospitalization
Coordinate discharge planning and facilitate referrals to appropriate community-based programs and social services
Serve as a patient advocate to ensure equitable access to care, services, and benefits
Maintain accurate, timely, and regulatory-compliant documentation in the electronic medical record (EMR)
Actively participate in interdisciplinary rounds, case conferences, and hospital-based quality improvement projects
At least 1 year of experience as an LMSW, preferably in a hospital inpatient med/surg setting
Strong communication and interpersonal skills
Proven ability to manage complex situations and collaborate in a fast-paced clinical environment
Master’s Degree in Social Work (MSW) from an accredited institution
LMSW License issued by the New York State Education Department (NYSED)
