Job Title: Licensed Practical Nurse (LPN) – Day / Evening / Night Shifts
Location: Far Rockaway, NY
Shift Options: Variable – Day, Evening, or Night (8-hour shifts)
Assignment Duration: 13+ Weeks
Weekly Hours: 40 hours/week
Pay Rate: $32 – $36 per hour
Employment Type: Contract
Deliver direct nursing care in accordance with resident care plans and physician directives
Administer medications and perform treatments following facility protocols and standards
Monitor and document residents’ health status, and promptly report changes to supervising RNs or providers
Provide skilled nursing procedures, including wound care, catheter maintenance, and more
Support residents with activities of daily living (ADLs)
Supervise and assist Certified Nursing Assistants (CNAs) during assigned shifts
Ensure compliance with infection control and safety protocols
Maintain accurate and timely nursing documentation
Promote a positive, respectful atmosphere for residents, families, and care team members
Active and unrestricted LPN license in the state of New York
BLS (Basic Life Support) certification
Minimum 1 year of experience in a long-term care or skilled nursing setting preferred
Strong clinical skills, attention to detail, and ability to prioritize in a fast-paced environment
