Job Description:
We are currently seeking a dependable and flexible Licensed Practical Nurse (LPN) for an immediate start. This contract position offers three 12-hour shifts per week, with a combination of night (NOC) and day shifts, totaling 36 hours weekly. The ideal candidate is experienced in long-term care and committed to delivering high-quality resident care.
Responsibilities:
Deliver professional nursing care in alignment with individualized care plans.
Administer medications and perform treatments accurately and on time.
Monitor resident conditions and report changes promptly.
Document all care and observations thoroughly and in compliance with regulations.
Communicate effectively with residents, families, and interdisciplinary team members.
Support the emotional and physical well-being of all residents.
Follow all facility protocols and state guidelines.
Requirements:
Active LPN license in Minnesota (or compact state license).
Minimum of 1 year of experience in a long-term care or nursing home setting preferred.
BLS (Basic Life Support) certification required.
Flexibility to work both 12-hour day and night shifts.
Immediate availability to start.
