Job Overview

We are seeking a highly skilled and motivated Registered Nurse – Case Manager (Travel) to join a dynamic care team in Hyannis, MA. This contract role offers the opportunity to work in a collaborative healthcare environment, focusing on effective discharge planning, utilization review, and care coordination.

Key Responsibilities

  • Perform daily discharge planning assessments and set priorities.

  • Complete InterQual® Initial Assessments within one business day.

  • Reassess patients every 3 days or as their medical status changes.

  • Determine appropriate level of care (Inpatient vs. Observation).

  • Issue HINN notices and reinstatement letters; manage appeals processes.

  • Conduct utilization reviews and collaborate with third-party payers.

  • Work closely with the interdisciplinary team to align patient care plans.

  • Serve as a patient advocate, facilitating communication with insurers.

  • Participate in daily unit rounds and interdisciplinary care conferences.

  • Coordinate safe and effective post-discharge transitions and referrals.

  • Apply and update Care Maps and clinical pathways.

  • Identify delays in care and contribute strategies to reduce length of stay.

  • Support coders by providing accurate clinical documentation.

  • Stay current on community resources and healthcare regulations.

Qualifications

  • Licensure: Active RN license in the Commonwealth of Massachusetts (required)

  • Education: BSN preferred (required for external applicants)

  • Certifications: Case Management Certification (CCM), CPUM, or specialty certification preferred

Experience Requirements

  • Minimum 3 years of acute care experience in a hospital setting within the past 5 years

  • Recent experience (within the past 4 years) in Discharge Planning and Utilization Review

  • Familiarity with InterQual® or a similar utilization review tool is required

  • A broad clinical background is strongly preferred
