Job Title: Travel Nurse – Inpatient Case Manager (RN)

Location: Hyannis, MA
Assignment Duration: 13 Weeks
Hours: Weekdays, 8:00 AM – 4:30 PM
Weekly Pay: $2,762 – $2,890

Position Overview:

This is a 13-week travel assignment for an experienced RN Case Manager supporting hospital-based care management. You'll ensure patients receive the right care at the right time with a focus on discharge planning and resource coordination.

Job Duties:

• Conduct timely admission assessments using InterQual® criteria
• Manage discharge planning and resource referrals
• Communicate with insurers to support continued stay and discharge planning
• Address financial, social, or behavioral health barriers to discharge
• Participate in daily interdisciplinary care rounds
• Assist with denial prevention through clinical documentation support
• Track at-risk days and coordinate post-acute needs
• Support appeals process and payer-required notifications
• Guide patients and families on transitions of care
• Maintain familiarity with community-based service providers

Experience & Certifications:

• Valid RN license (Massachusetts)
• 3+ years in acute care or case management
• BSN preferred
• Certified Case Manager (CCM) a plus
• EMR and InterQual® experience required
• Strong problem-solving and interpersonal skills
