Job Title: RN Case Manager – Acute Care (Travel)

Location: Hyannis, MA
Contract Length: 13 Weeks
Schedule: Mon–Fri, 8:00 AM to 4:30 PM
Compensation: $2,762–$2,890/week

Role Summary:

Join a dynamic care coordination team as a Travel RN Case Manager. This role focuses on assessing patient needs, optimizing utilization of services, and ensuring smooth care transitions for hospitalized patients.

Primary Responsibilities:

• Screen and assess patients on admission for discharge and care planning
• Use InterQual® to conduct utilization reviews within defined timelines
• Collaborate with medical teams to support timely and appropriate discharges
• Address payer requirements and communicate appeal status
• Identify patients with high-risk needs and connect them to appropriate resources
• Ensure timely response to insurance documentation needs
• Guide discharge planning and coordination with external providers
• Track patient progress during interdisciplinary team discussions
• Monitor care quality and contribute to continuous improvement efforts
• Educate patients and families on post-discharge care and follow-up
• Stay current on regulations, quality standards, and hospital policies

Required Qualifications:

• MA RN license in good standing
• Bachelor’s degree in nursing preferred
• Minimum 3 years in hospital or case management nursing
• InterQual® or comparable experience essential
• Knowledge of discharge protocols and payer guidelines
• Strong documentation and EHR proficiency
• Certified Case Manager (CCM) or related credential a plus
