Location: Utica, NY 13502
Shift: 10:30 PM – 6:30 AM (Every other weekend required; must float to all units)
Weekly Hours: 37.5 hours
Assignment Type: Long-Term Care (LTC)

Position Overview:

We are seeking a dedicated and experienced Licensed Practical Nurse (LPN) to join a long-term care team in Utica, NY. This role is vital in delivering compassionate, patient-centered care to residents, supporting their daily health and personal needs, and collaborating closely with other healthcare professionals.

Key Responsibilities:

  • Provide direct nursing care including administering prescribed medications and treatments

  • Monitor and document residents’ health status and promptly report changes to supervising RN or physician

  • Assist residents with Activities of Daily Living (ADLs) such as bathing, dressing, toileting, feeding, and mobility

  • Record and track vital signs, intake/output, weight, and pain levels

  • Implement individualized care plans tailored to resident needs

  • Collaborate with an interdisciplinary care team including RNs, CNAs, physicians, and therapists

  • Collect specimens for lab testing and assist in basic diagnostic procedures

  • Maintain accurate and timely documentation within electronic medical records (EMR/EHR)

  • Observe residents for signs of infection, pressure ulcers, or other complications and take preventative or corrective action

  • Provide emotional support and health education to residents and their families

Qualifications:

  • Active LPN license in the state of New York

  • Minimum 1 year of experience in a long-term care or skilled nursing facility preferred

  • Strong observational and critical thinking skills

  • Proficient in using electronic medical record systems

  • Compassionate, dependable, and able to work independently or as part of a team

  • Ability and willingness to float across all units as needed

  • Must be available to work every other weekend
