Schedule: 12-hour shifts | 6:00 AM – 6:00 PM | 38–48 hours per week

Position Overview

We are currently seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our healthcare team in Flagstaff, AZ. This position is responsible for providing hands-on nursing care, including medication administration, clinical treatments, and patient monitoring in a fast-paced clinical environment. Ideal candidates will have experience in geriatric or long-term care and a strong commitment to patient-centered service.

Core Responsibilities

  • Administer medications and perform physician-ordered treatments in accordance with the LPN scope of practice.

  • Provide care including wound treatment, catheter and tracheostomy care, postural drainage, and enteral feeding.

  • Monitor, document, and report patient responses to medications and treatments.

  • Ensure proper inventory levels of medical supplies and medications.

  • Participate in shift change reporting and maintain effective communication with the care team and patient families.

  • Conduct initial nursing assessments for new admissions.

  • Assist with development and updates of care plans, MDS assessments, and quarterly reviews.

  • Maintain accurate documentation of patient diagnoses, observations, and care interventions.

  • Contribute to the facility’s Quality Assurance and Performance Improvement (QAPI) initiatives.

  • Attend staff meetings and participate in ongoing training as needed.

Qualifications

  • Active LPN license in the state of Arizona (or multistate compact license)

  • Current BLS (Basic Life Support) certification

  • Minimum 1 year of LPN experience preferred, ideally in geriatric or long-term care settings

  • Strong organizational and interpersonal skills
