Job Title: Licensed Practical Nurse (LPN)

Pay Rate: Starting at $30/hour (commensurate with experience)
Schedule: 12-hour shifts | 6:00 AM – 6:00 PM | 38–48 hours per week
We are currently seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our healthcare team in Flagstaff, AZ. This position is responsible for providing hands-on nursing care, including medication administration, clinical treatments, and patient monitoring in a fast-paced clinical environment. Ideal candidates will have experience in geriatric or long-term care and a strong commitment to patient-centered service.
Administer medications and perform physician-ordered treatments in accordance with the LPN scope of practice.
Provide care including wound treatment, catheter and tracheostomy care, postural drainage, and enteral feeding.
Monitor, document, and report patient responses to medications and treatments.
Ensure proper inventory levels of medical supplies and medications.
Participate in shift change reporting and maintain effective communication with the care team and patient families.
Conduct initial nursing assessments for new admissions.
Assist with development and updates of care plans, MDS assessments, and quarterly reviews.
Maintain accurate documentation of patient diagnoses, observations, and care interventions.
Contribute to the facility’s Quality Assurance and Performance Improvement (QAPI) initiatives.
Attend staff meetings and participate in ongoing training as needed.
Active LPN license in the state of Arizona (or multistate compact license)
Current BLS (Basic Life Support) certification
Minimum 1 year of LPN experience preferred, ideally in geriatric or long-term care settings
Strong organizational and interpersonal skills
