A reputable healthcare organization in South Dennis, MA is seeking a dedicated and experienced Registered Nurse (RN) Case Manager to join its Home Health team. This is an excellent opportunity for a compassionate professional to provide high-quality, patient-centered care in a home-based setting.

Position Overview:
The RN Case Manager will be responsible for delivering skilled nursing care and managing a caseload of patients in their homes. This role requires strong clinical judgment, effective communication, and a commitment to improving patient outcomes through individualized care planning and coordination.

Key Responsibilities:

  • Provide direct, skilled nursing care to patients in accordance with physician orders and care plans.

  • Develop and implement individualized care plans in collaboration with interdisciplinary healthcare teams.

  • Monitor patient conditions and adjust care plans as necessary to ensure optimal outcomes.

  • Educate patients and their families on disease management, medications, and home care procedures.

  • Maintain accurate and timely documentation in compliance with organizational policies and state/federal regulations.

  • Coordinate with physicians, caregivers, and other healthcare professionals to ensure continuity of care.

Qualifications:

  • Active Registered Nurse (RN) license in the state of Massachusetts.

  • Minimum of 1 year of clinical nursing experience; home health or hospice experience preferred.

  • Strong clinical assessment and critical thinking skills.

  • Excellent interpersonal, communication, and organizational abilities.

  • Reliable transportation and a valid driver’s license are required.
