Registered Nurse (RN) – Long-Term Care
Location: Millbrook, NY
Position Summary:
The Registered Nurse (RN) provides and coordinates comprehensive nursing care for residents in a long-term care setting. The RN ensures high standards of patient care, promotes resident safety and well-being, and supports the interdisciplinary team in delivering quality health services.
Key Responsibilities:
Perform nursing assessments and contribute to the development and implementation of individualized care plans.
Administer medications, treatments, and procedures in accordance with physician orders and applicable regulations.
Monitor resident health status and respond to changes in condition.
Collaborate with physicians, families, and interdisciplinary team members to ensure effective care coordination.
Supervise and support Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), and other staff.
Maintain accurate, timely documentation and reports in resident medical records.
Participate in care planning meetings, quality improvement initiatives, and policy compliance reviews.
Adhere to infection control protocols, safety guidelines, and organizational procedures.
Educate residents and families on health conditions, medications, and ongoing care needs.
Provide compassionate, person-centered care that respects residents' dignity and preferences.
Qualifications:
Active and unencumbered RN license in the state of practice.
BLS/CPR certification required.
Experience in long-term care, geriatric, or rehabilitative nursing preferred.
Strong clinical assessment, communication, and leadership skills.
Ability to work collaboratively in a team-based environment.
