Location: Syracuse, NY 13210

Shift: Days (8-hour shifts, 5 days per week)

Duration: 13 weeks (with possible extension)

Pay: $68 - $85 per hour

Job Overview:

Seeking an experienced Registered Nurse (RN) for Inpatient Case Management in an acute care hospital. This role focuses on Utilization Review, Clinical Documentation Improvement (CDI), and Discharge Planning to ensure safe and efficient patient transitions. Ideal candidates should have strong hospital case management experience and proficiency with EPIC EMR.

Responsibilities:

  • Manage inpatient case management and discharge planning to ensure continuity of care.

  • Conduct utilization reviews and clinical documentation assessments to support insurance approvals.

  • Coordinate patient care transitions, working closely with interdisciplinary teams.

  • Ensure compliance with hospital policies, payer requirements, and state regulations.

  • Utilize EPIC EMR to review patient records and improve documentation accuracy.

  • Collaborate with social workers, physicians, and post-acute care providers to facilitate timely discharges.

Requirements:

  • Active New York State RN License (Required)

  • BLS Certification (Required)

  • Minimum 1.5 years of recent inpatient case management experience in an acute care hospital (Required)

  • Experience with EPIC EMR (Preferred)

  • Strong knowledge of inpatient utilization review, CDI, and discharge planning

  • Excellent communication, critical thinking, and problem-solving skills

Preferred Experience:

  • Emergency Department (ED) Case Management experience (Preferred)

  • Experience in hospitals of equal or greater size or trauma centers

  • Managing high-acuity patient care transitions
