Job Title: Healthcare Social Worker - LMSW

Duties:

• Assess and address the social and emotional needs of patients in a healthcare environment.
• Develop and implement care plans, including discharge strategies and resource allocation.
• Provide crisis intervention and support for patients and families during medical care.
• Coordinate with external agencies to ensure access to financial and social support services.
• Engage in educational development for healthcare professionals on social work theories and applications.
• Participate in multidisciplinary team discussions to improve patient outcomes.

Skills Required:

• Experience in medical social work, case management, and community resource navigation.
• Licensed Master Social Worker (LMSW) certification required; LCSW preferred.
• Proficiency in basic MS Office tools and documentation processes.

Education:

• Master’s Degree in Social Work from an accredited program.

Certifications & Licenses:

• Valid LMSW certification required.

Stipend:

• Weekly: $1,650 - $2,250
• Monthly: $6,600 - $9,000
