Job Title: Social Work Practitioner - Patient Care

Duties:

• Conduct comprehensive psychosocial assessments for patients and their families.
• Deliver individualized counseling and support to address barriers to care.
• Facilitate placement in appropriate care facilities, rehabilitation programs, and community services.
• Participate in quality improvement initiatives to enhance social work services.
• Develop and maintain collaborative relationships with healthcare and community providers.
• Maintain detailed and accurate patient records for compliance and reporting.

Skills Required:

• Proficiency in social work practices and patient advocacy.
• Licensed Master Social Worker (LMSW); familiarity with community resources.
• High-level analytical and interpersonal skills for effective care coordination.

Education:

• Master’s Degree in Social Work from an accredited institution.

Certifications & Licenses:

• Active LMSW license required.

Stipend:

• Weekly: $1,600 - $2,200
• Monthly: $6,400 - $8,800
