Job Title: Community Social Worker - Medical Support

Duties:

• Evaluate and address social and environmental factors impacting patient care outcomes.
• Develop and execute strategies for counseling and support services tailored to patient needs.
• Serve as a liaison between patients and community agencies, ensuring access to resources.
• Coordinate with healthcare teams to streamline discharge planning and patient follow-up care.
• Document assessments, interventions, and progress in accordance with healthcare protocols.
• Facilitate educational workshops for healthcare staff and students on social work practices.

Skills Required:

• Knowledge of social work methodologies and healthcare operations.
• Licensed Master Social Worker (LMSW) with demonstrated experience in patient services.
• Ability to manage complex cases and navigate community and governmental programs.

Education:

• Master’s Degree in Social Work required.

Certifications & Licenses:

• Current LMSW certification; LCSW preferred for advanced cases.

Stipend:

• Weekly: $1,550 - $2,150
• Monthly: $6,200 - $8,600
