Job Title: Clinical Social Worker - Healthcare Services
Duties:
Conduct social, psychological, and environmental assessments for patients and families.
Provide group and individual therapy sessions addressing challenges related to medical care.
Assist patients in accessing community services, financial aid, and specialized resources.
Plan and manage discharge transitions, ensuring proper placement in care facilities.
Collaborate with healthcare teams to address patients’ social and emotional needs effectively.
Contribute to the training and supervision of less experienced social workers and students.
Skills Required:
Expertise in social work processes and healthcare resources.
Licensed Master Social Worker (LMSW); LCSW preferred for some roles.
Proficient in MS Office Suite with strong analytical and communication skills.
Education:
Master’s Degree in Social Work from an accredited institution.
Certifications & Licenses:
Active LMSW certification; additional LCSW preferred for leadership roles.
