Job Title: Contracted Social Worker - Patient Services

Duties:

• Assess patients’ social, psychological, and environmental conditions through interviews and medical record reviews.
• Develop individualized care plans and provide counseling to patients and families facing challenges related to illness and disabilities.
• Coordinate with community resources for financial assistance, rehabilitation, and support services.
• Facilitate discharge planning for patients requiring specialized or extended care, including placement in appropriate facilities.
• Actively participate in interdisciplinary team meetings to enhance patient care and communicate identified social and emotional challenges.
• Maintain updated knowledge of community resources, social work practices, and regulations.

Skills Required:

• Understanding of social work theories, techniques, and healthcare systems.
• Licensed Master Social Worker (LMSW) with experience in medical or healthcare settings.
• Strong interpersonal and analytical skills to support patient assessment and counseling.

Education:

• Master’s Degree in Social Work from an accredited institution.

Certifications & Licenses:

• Active LMSW license; some positions may require LCSW certification.

Stipend:

• Weekly: $1,450 - $2,050
• Monthly: $5,800 - $8,200
