Job Title: Social Work Coordinator - LMSW

Duties:

• Conduct psychosocial evaluations and develop patient care plans.
• Support patient transitions from hospital to home or other care facilities.
• Manage documentation using EPIC, ACM, and Careport systems.
• Provide guidance and counseling to patients and families in crisis.
• Work collaboratively with interdisciplinary teams to optimize patient outcomes.

Skills Required:

• Hospital-based social work experience with EPIC and ACM systems.
• Proficiency in discharge planning and emergency response.

Education:

• Master’s Degree in Social Work and a valid LMSW license.

Stipend:

• Weekly: $1,600 - $2,200
• Monthly: $6,400 - $8,800
