• Responsibilities: The Medication Therapy Management Pharmacist is responsible for evaluating and optimizing the pharmacologic treatment of patients with chronic or complex conditions. This role requires conducting medication reviews, identifying potential drug-related problems, and making therapeutic recommendations to healthcare providers. You will also educate patients about their medications and help them manage adherence to their prescribed regimens, with an emphasis on improving clinical outcomes and preventing adverse drug events.
• Required Skills: Strong clinical judgment, excellent communication skills, and the ability to analyze patient medication regimens for efficacy and safety.
• Qualifications: Pharm.D., state licensure, certification in medication therapy management preferred.
