Patient Medication Management Specialist
Responsibilities: The Patient Medication Management Specialist is responsible for overseeing the therapeutic management of medications for patients, ensuring they are on the best possible treatment regimens. This role includes evaluating patient prescriptions, monitoring patient progress, offering medication counseling, and recommending adjustments based on clinical needs. You will collaborate with physicians and healthcare providers to track patient outcomes and adjust care plans as necessary to optimize therapy.
Required Skills: In-depth knowledge of pharmacology, excellent problem-solving skills, ability to work with patients and healthcare teams to manage care plans.
Qualifications: Pharm.D., board certification in pharmacotherapy.
