Pharmacy Care Coordinator
Responsibilities: As a Pharmacy Care Coordinator, you will take an active role in coordinating the medication management process for patients in hospital and outpatient settings. You will work closely with physicians, nurses, and other healthcare professionals to optimize medication therapy, ensure patient adherence to prescribed treatments, and help improve overall health outcomes. You will also be responsible for providing medication counseling and monitoring for potential drug interactions and side effects.
