Shift: Day shift, 8:30 AM - 5:00 PM

Job Summary:
We are seeking a compassionate and experienced Registered Nurse (RN) to provide professional nursing care to patients in their homes. The RN will be responsible for assessing, developing, implementing, and evaluating individualized care plans, ensuring high-quality patient outcomes. This role requires adherence to clinical guidelines, organization policies, and regulatory standards.

Key Responsibilities:

• Provide skilled nursing care in a home setting following physician orders.
• Develop and implement individualized patient care plans, ensuring timely updates.
• Monitor and document patient progress, addressing goals and discharge planning.
• Ensure safe and effective care delivery through clinical judgment and adherence to professional standards.
• Utilize interdisciplinary team collaboration and accurately document patient care activities.
• Maintain compliance with insurance authorizations and payor source guidelines.
• Perform medication reconciliation and coordinate care with physicians.
• Educate patients and caregivers on treatment plans and home care practices.
• Act as a clinical supervisor for home health aides, conducting required supervisory visits.
• Communicate critical patient updates to the healthcare team for effective care coordination.
• Maintain compliance with documentation standards and regulatory requirements.
• Participate in ongoing professional development and incorporate best practices into patient care.

Required Qualifications:

• Active NJ State Professional Registered Nurse license or multi-state compact license.
• Bachelor of Science in Nursing (BSN) from an accredited school.
• Minimum of two years of relevant nursing experience.
• Acute care/hospital experience required.
• BLS certification (AHA).
• Experience with OASIS documentation.
• Familiarity with Epic electronic health records (EHR).

Preferred Qualifications:

• Previous experience in certified home care settings.

Note: 6 months contract preferred
