Job Description:

We are seeking a dedicated and experienced Wound Care Nurse to provide specialized care for patients with wounds, ostomies, and continence-related conditions. This role involves conducting patient assessments, developing individualized treatment plans, and providing education to patients and healthcare staff. While primarily office-based (70%), this position also requires travel to patient homes across Manhattan, Brooklyn, Bronx, and Queens for wound care consults.

Key Responsibilities:

• Perform comprehensive wound, ostomy, and continence assessments and develop individualized care plans.
• Recommend and implement appropriate medications and treatment strategies.
• Collaborate with the healthcare team to ensure optimal patient outcomes.
• Educate patients, families, and staff on wound care best practices and complication prevention.
• Conduct staff education programs, including orientation, in-service training, and continuing education.
• Assess and document patient responses to treatment and adjust care plans accordingly.
• Participate in quality improvement initiatives within the department and hospital.
• Ensure compliance with data security and patient confidentiality policies.
• Identify and connect patients with community resources to support ongoing care.
• Manage rented wound care equipment, including specialty beds and negative pressure devices.

Qualifications:

Required:

• Active New York State RN License (Primary Source Verification).
• AHA BLS (Basic Life Support) Certification.
• Minimum 2 years of medical/surgical experience with a focus on wound and colostomy care.
• Current certification in at least one of the following:
  • Certified Wound Ostomy Continence Nurse (CWOCN) – WOCNCB
  • Certified Wound Ostomy Nurse (CWON) – WOCNCB
  • Certified Wound Care Nurse (CWCN) – WOCNCB
  • Certified Wound Specialist (CWS) – ABWM
  • Wound Care Certified (WCC) – NAWCO

Education Requirements:

• Bachelor’s Degree in Nursing (BSN) required.

Schedule & Work Expectations:

• 40 hours per week | 8-hour shifts | 5 days per week.
• 70% office-based; 30% travel to patient homes across Manhattan, Brooklyn, Bronx, and Queens.
• Expected to handle 15-20 wound care consults per day.
