Registered Nurse – Geriatric Triage Nurse

Qualifications:

• Current RN license with at least 2 years of experience in geriatric or long-term care.
• BLS certification required; Geriatric Nursing Certification (RN-BC) preferred.
• Experience in assessing cognitive decline, fall risk, and chronic disease management.

Duties:

• Assess elderly patients for medical concerns, including infections, dehydration, and falls.
• Evaluate cognitive function and recognize early signs of dementia and delirium.
• Provide guidance on chronic disease management and medication adherence.
• Prioritize care for high-risk geriatric patients and facilitate emergency interventions.
• Educate families on long-term care planning and support resources.
• Collaborate with geriatricians, social workers, and rehabilitation specialists.
• Maintain compliance with geriatric care protocols and patient safety measures.
