Job Description:

We are seeking a dedicated Licensed Practical Nurse (LPN) for a local-only contract position at a long-term care (LTC) facility in Far Rockaway, NY. This is a 13+ week assignment with 8-hour - day, night & evening shifts. The ideal candidate will have experience in geriatric care and a strong commitment to providing high-quality nursing services to residents.

Responsibilities:

• Provide direct nursing care to residents in accordance with their care plans
• Administer medications and treatments as prescribed
• Monitor patient conditions and report changes to the RN or physician
• Assist with daily living activities, such as bathing, dressing, and mobility support
• Maintain accurate and up-to-date patient records
• Collaborate with the healthcare team to ensure the best possible resident outcomes
• Adhere to all facility policies and state/federal regulations

Requirements:

• Active LPN License in the state of New York
• Minimum of 1 year experience in a long-term care or nursing home setting
• Must be local to the Far Rockaway, NY area
• Ability to work night, day & evening shifts (8-hour shifts)
• Strong communication and teamwork skills
• CPR/BLS certification (preferred)
