Registered Nurse – Chronic Care Case Manager

Qualifications:

• RN license with experience in chronic disease management.
• At least 2 years of experience in case management or population health.
• Strong understanding of chronic conditions such as diabetes, heart disease, and COPD.
• Excellent patient education and communication skills.
• Ability to work with interdisciplinary healthcare teams.

Duties:

• Develop care plans for patients with chronic health conditions.
• Educate patients on disease self-management and lifestyle modifications.
• Monitor patient adherence to treatment plans and follow-up care.
• Coordinate referrals to specialists and community health resources.
• Support medication management and treatment compliance.
• Utilize data-driven approaches to improve patient outcomes.
• Advocate for patient wellness and preventive care strategies.

Pay:

• Medium weekly pay range: $1,950 - $2,350
