Registered Nurse – Case Management Specialist

Qualifications:

• Active Registered Nurse (RN) license in the state of employment.
• Bachelor’s degree in Nursing (BSN) preferred.
• Minimum of 2 years of experience in case management, discharge planning, or care coordination.
• Certified Case Manager (CCM) credential preferred.
• Strong knowledge of healthcare regulations, insurance, and community resources.
• Excellent communication and problem-solving skills.

Duties:

• Assess patient needs and develop individualized care plans.
• Coordinate patient care across multiple healthcare providers and specialists.
• Facilitate discharge planning and ensure continuity of care.
• Educate patients and families on treatment plans and community resources.
• Monitor patient progress and adjust care plans as needed.
• Collaborate with insurance providers for coverage and authorizations.
• Maintain accurate case management documentation.

Pay:

• Medium weekly pay range: $1,950 - $2,400
