Registered Nurse – Transitional Care Unit (TCU) Nurse 

Qualifications: 

• Active Registered Nurse (RN) license in the state of employment. 

• Bachelor’s degree in Nursing (BSN) preferred. 

• Minimum of 2 years of experience in a transitional care, rehabilitation, or medical-surgical setting. 

• Advanced Cardiac Life Support (ACLS) and Basic Life Support (BLS) certification required. 

• Strong knowledge of post-acute care, rehabilitation therapies, and patient discharge planning. 

• Experience in wound care, IV therapy, medication management, and mobility assistance. 

Duties: 

• Provide comprehensive nursing care to patients transitioning from hospital to home or long-term care. 

• Monitor patient progress and collaborate with interdisciplinary teams to support rehabilitation goals. 

• Administer medications, IV fluids, and wound care treatments as prescribed. 

• Educate patients and families on recovery, medications, and lifestyle changes. 

• Assess for and respond to complications such as infections, falls, and medication reactions. 

• Assist with discharge planning and coordination of follow-up care with outpatient providers. 

• Maintain accurate patient documentation and ensure compliance with hospital policies.
