Job Title: Licensed Master Social Worker - Emergency Department

Duties:

Deliver social work services to patients and families in a high-paced ER environment.
Assess psychosocial challenges and provide crisis intervention support.
Coordinate patient discharge planning and follow-up care using ACM and Careport.
Maintain accurate and timely records in EPIC.
Collaborate with healthcare teams to address patient needs efficiently.

Skills Required:

ER experience with expertise in EPIC and care management systems.
Familiarity with ACM and Careport for discharge coordination.

Education:

Master’s in Social Work (MSW) and an active LMSW license.
