Job Title: RN Case Manager

Location: San Francisco, CA
Shift: 8:00 AM to 4:30 PM

Job Description:

 The RN Case Manager will play a critical role in coordinating and managing patient care to ensure optimal outcomes. This role requires a strong clinical background, excellent communication skills, and the ability to work collaboratively with patients, families, and multidisciplinary teams.

Key Responsibilities:

• Case Management: Manage the care of patients by developing, implementing, and evaluating individualized care plans. Ensure that patients receive the appropriate medical care, services, and resources.
• Care Coordination: Collaborate with physicians, social workers, and other healthcare professionals to coordinate the delivery of care and services for patients.
• Patient Education: Provide education to patients and their families on managing their conditions, treatment plans, and healthcare resources.
• Discharge Planning: Assist in planning patient discharges, ensuring smooth transitions from hospital to home or other facilities.
• Documentation: Maintain accurate and up-to-date patient records, including care plans, progress notes, and communication with healthcare providers.

Qualifications:

• Education: A bachelor’s degree in nursing (BSN) is preferred, and an associate’s degree in nursing (ADN) is required.
• Experience: Minimum of 2 years of experience as a registered nurse in a clinical setting. Experience in case management is highly preferred.

Licenses/Certifications:

• Active Registered Nurse (RN) license in the state of California.
• BLS (Basic Life Support) certification from the American Heart Association (AHA).
