Job Title: Licensed Practical Nurse (LPN) Location: Yuma, AZ 85364 Shift: 6:00 AM - 6:00 PM
Job Summary:
The Licensed Practical Nurse (LPN) will provide compassionate and high-quality nursing care to residents in a long-term care facility. This position requires an individual who is skilled in administering direct patient care, performing assessments, assisting with treatments, and collaborating with the interdisciplinary healthcare team. The LPN ensures residents' comfort, health, and safety during their daily care routines.
Responsibilities:
Administer medications and treatments as prescribed by the attending physician.
Monitor and document residents' vital signs, including temperature, blood pressure, pulse, and respiratory rates.
Assist with daily activities of living, including bathing, dressing, grooming, and feeding as needed.
Perform and document assessments of residents' physical and emotional health and report any changes to the supervising nurse or physician.
Provide wound care, catheter care, and other clinical procedures as needed.
Ensure all resident care is provided in a safe, timely, and respectful manner.
Documentation:
Accurately document all nursing assessments, treatments, medications, and resident progress in electronic health records (EHR) or paper charts.
Maintain and update care plans as required, ensuring all information is current and accurate.
Document any incidents or changes in resident conditions promptly.
Education:
Graduation from an accredited Practical Nursing program.
Current state licensure as a Licensed Practical Nurse (LPN).
Experience:
Previous experience in long-term care, nursing home, or geriatric settings preferred.
Familiarity with electronic health records (EHR) systems.
