Job Title: Licensed Practical Nurse (LPN) – Medical/Surgical Nursing
Location: Lagrange, GA
Shift: 6:45 AM - 6:45 PM
Shifts per Week: 3
Hours per Shift: 12-hour shifts

Position Overview:

The Licensed Practical Nurse (LPN) functions under the supervision of a Registered Nurse (RN) or Manager and operates within the LPN Scope of Practice. The LPN provides direct patient care for individuals with medical/surgical needs, delivering age-appropriate interventions that meet physical, emotional, developmental, psychosocial, and educational needs. This role requires the coordination and delivery of high-quality care while participating in call rotations and weekend visits as needed. The LPN may also serve as a preceptor for new staff.

Key Responsibilities:

  • Assess and manage patient care needs by following physician orders with RN oversight.
  • Perform routine nursing procedures and interventions as directed.
  • Meet established productivity and care quality standards.
  • Work collaboratively with RNs and other healthcare team members to coordinate patient care.
  • Report changes in patient condition to the RN, therapist, or clinical supervisor.
  • Assist the RN in developing and revising individualized care plans based on patient needs and physician orders.
  • Educate patients and caregivers to foster self-care or provide support in the home environment.
  • Report any concerns related to patient or staff safety to authorities or agency leadership.
  • Assist in the orientation and education of new employees and nursing students as needed.
  • Participate in continued education, in-services, and special projects.
  • Maintain current clinical knowledge and practices relevant to home health patient care.
  • Accurately complete all required documentation within the established timeframe.
  • Apply knowledge of regulatory guidelines appropriately in practice.
  • Participate in care team meetings and conferences with co-workers.
  • Check voicemail and secure messaging a minimum of once daily to ensure timely communication.

Required Minimum Qualifications:

Education: High School Diploma or equivalent; Certificate in Nursing from an accredited program.

License/Certification:

  • Licensed Practical Nurse (LPN) certification upon hire.
  • Basic Life Support (BLS) certification.
  • Valid Georgia driver’s license (preferred).
  • Private vehicle insurance (preferred).

Experience:

  • Minimum 1 year of medical/surgical nursing practice in an acute care setting is preferred.

Skills:

  • Strong leadership, problem-solving, and organizational skills.
  • Excellent assessment and clinical skills.
  • Proficient written and verbal communication skills.
  • Ability to work independently in a home health setting and follow directions to patient homes.
  • Proficiency with Windows-based computer systems and patient care software.
