Job Title: Registered Nurse Care Manager
Location: York, ME
Type: Contract
Duration: 13 Weeks (With Strong Possibility of Extension or Permanent Placement)
Schedule:  Variable Schedule
• Option 1: Monday–Friday, 8:30 AM – 4:30 PM
• Option 2: Four (4) 10-hour shifts, 7:30 AM – 5:00 PM 


Pay Rate: $2,100/week - $2,500/week (Based on Eligibility and Experience)

Position Overview:

Navitas Healthcare, LLC is seeking a Registered Nurse Care Manager for an exciting Contract opportunity in York, ME.

Responsibilities

  • Conduct comprehensive patient assessments and identify discharge planning needs.

  • Coordinate care transitions and secure appropriate post-acute services and resources.

  • Collaborate with interdisciplinary teams during daily rounds, care conferences, and care planning meetings.

  • Facilitate utilization review activities and concurrent case management.

  • Work with insurance providers to obtain authorizations and approvals.

  • Coordinate transportation, durable medical equipment, home health services, and community resources.

  • Monitor patient length of stay and support efficient resource utilization.

  • Ensure compliance with hospital policies, payer requirements, and regulatory standards.

  • Maintain accurate documentation within electronic healthcare systems.

  • Educate patients and families regarding care plans and discharge processes.

Required Qualifications

  • Active Registered Nurse (RN) License.

  • BLS/BCLS Certification.

  • Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) required.

  • Minimum 3 years of clinical nursing experience required.

  • Strong utilization review and discharge planning experience.

  • Must have recent utilization review and discharge planning experience.

  • Excellent communication, organizational, and critical-thinking skills.

  • Ability to work collaboratively with multidisciplinary healthcare teams.

  • Knowledge of healthcare reimbursement methodologies and quality measures.

  • Current active state license required at submission.

Preferred Qualifications

  • CCM (Certified Case Manager) Preferred.

  • ACM (Accredited Case Manager) Preferred.

  • CCMA Preferred.

  • Preferred backgrounds include Medical-Surgical Nursing, Hospital Case Management, Home Health Care, and Utilization Review.

  • Experience coordinating community resources and post-acute care services preferred.
