Position Overview

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to provide high-quality nursing care in a Long-Term Care setting. The LPN will work collaboratively with the interdisciplinary healthcare team to support resident health, safety, and overall well-being.

Required Qualifications

Licensure

  • Active New York LPN License
  • License verification through Nursys

Certifications

  • Current BLS Certification

Preferred Experience

  • Long-Term Care (LTC) experience
  • Skilled Nursing Facility experience

Key Responsibilities

Resident Care

  • Provide direct nursing care to long-term care residents
  • Monitor resident conditions and report significant changes
  • Assist with development and implementation of individualized care plans
  • Monitor vital signs, symptoms, and overall health status

Medication & Treatment Management

  • Administer medications and treatments according to physician orders
  • Perform nursing procedures within LPN scope of practice
  • Document all care accurately and timely

Communication & Collaboration

  • Collaborate with nurses, physicians, therapists, and support staff
  • Communicate resident concerns and status updates effectively
  • Participate in interdisciplinary care planning activities

Documentation & Compliance

  • Maintain accurate clinical records
  • Adhere to infection control and safety protocols
  • Ensure compliance with facility policies and regulatory requirements
