Registered Nurse (RN) – Home Health

Location: South Dennis, MA

Contract Length: 13 Weeks

Job Summary

"Navitas Healthcare, LLC" is seeking an experienced Home Health Registered Nurse (RN) to provide skilled nursing care to patients in their homes. The RN will perform comprehensive assessments, develop individualized care plans, coordinate patient care, and ensure quality outcomes while maintaining compliance with home health standards and regulations.

Required Qualifications

  • Active Massachusetts RN License

  • Current CPR Certification

  • Minimum 1 Year Nursing Experience Required

    • OR 6 Months Nursing Experience with BSN

  • Successful Completion of Medication Test Required

  • Strong Assessment and Care Coordination Skills

Preferred Qualifications

  • Home Health Experience

  • IV Therapy Experience

  • BSN Preferred

  • Computer and EMR Experience

Responsibilities

  • Perform comprehensive patient assessments upon admission

  • Develop, implement, and revise individualized plans of care

  • Coordinate interdisciplinary services and patient care needs

  • Monitor patient progress and evaluate care effectiveness

  • Participate in interdisciplinary case conferences

  • Supervise Home Health Aides and document required oversight

  • Communicate significant patient condition changes to physicians and care teams

  • Obtain and clarify physician orders for services provided

  • Complete admissions, recertifications, and discharge documentation

  • Provide patient and caregiver education

  • Maintain clear, accurate, and timely clinical documentation

  • Coordinate patient discharge planning and community resource referrals

  • Ensure compliance with quality standards and home health regulations
