Occupational Therapist (OT) – Acute Care Hospital

Location: Santa Rosa, CA

Job Summary

"Navitas Healthcare, LLC" is seeking an experienced Occupational Therapist (OT) to join an Acute Care Hospital team in Santa Rosa, CA. The ideal candidate will have experience evaluating and treating patients with acute and chronic conditions while helping them improve functional independence and quality of life.

Key Responsibilities

• Perform comprehensive patient assessments and evaluations
• Develop and implement individualized treatment plans
• Provide therapy for patients with acute and chronic disorders
• Assist patients in improving functional activities and daily living skills
• Conduct fall risk assessments and implement safety interventions
• Maintain compliance with infection prevention protocols
• Complete accurate billing and clinical documentation
• Collaborate with physicians, nurses, and rehabilitation teams
• Educate patients and families regarding treatment plans and recovery goals
• Follow National Patient Safety Goals and Core Measures

Requirements

• Minimum 1 year of Occupational Therapy experience
• Acute Care Hospital experience preferred
• Experience with:
  • Infection Prevention
  • Fall/Risk Assessment
  • Billing
  • Acute Disorders
  • Chronic Disorders
  • Functional Activities
  • Patient Assessment
• Active California OT License required
• EPIC experience preferred
• Ability to float between like units as needed
