Nurse Practitioner (NP) – Long-Term Care

Pay Range: $100,000 – $185,000/year (Based on Experience)
We are seeking a licensed Nurse Practitioner (NP) to provide advanced clinical care in a long-term care setting. The ideal candidate will be experienced in managing complex acute and chronic conditions, leading care coordination, and collaborating with interdisciplinary healthcare teams.
Perform comprehensive patient assessments and physical examinations
Diagnose and manage acute and chronic conditions
Develop and implement evidence-based treatment plans
Prescribe medications and controlled substances (DEA required)
Order, perform, and interpret diagnostic tests and labs
Collaborate with physicians and interdisciplinary care teams
Provide patient education, counseling, and preventive care
Document all patient care per regulatory standards
Participate in emergency and critical care response situations
Lead care coordination and discharge planning
Active Nurse Practitioner (NP) license (NY State)
DEA registration required
Long-term care experience preferred
Strong clinical judgment and patient management skills
EMR documentation experience preferred
Bilingual (Spanish preferred but not required)
