Job Title: Registered Nurse (RN) – Case Manager

Location: Apple Valley, California

Contract Duration: 13 Weeks

Requirements

• Active California RN License required (must be active by start date)
• Minimum 2 years of Acute Care Case Management and/or Discharge Planning experience required
• Acute Care Hospital experience required
• Behavioral Health Case Management experience alone does not qualify
• Experienced travelers only; first-time travelers will not be considered

Schedule

• Day Shift
• Weekend coverage required based on department needs (approximately every other weekend)

Job Description

We are seeking an experienced RN Case Manager to coordinate patient care, discharge planning, and utilization management activities within an acute care hospital setting. The ideal candidate will possess strong clinical judgment, discharge planning expertise, and the ability to collaborate effectively with multidisciplinary healthcare teams to ensure safe and efficient patient transitions.

Responsibilities

• Coordinate and manage patient care throughout hospitalization
• Develop and implement discharge plans to support safe patient transitions
• Collaborate with physicians, nurses, social workers, and interdisciplinary care teams
• Assess patient needs and identify appropriate post-acute care resources
• Facilitate communication between patients, families, providers, and community agencies
• Monitor patient progression and address barriers to discharge
• Ensure compliance with hospital, regulatory, and payer requirements
• Maintain accurate and timely documentation of case management activities
• Participate in utilization review and resource management processes as needed
