Job Title: Licensed Practical Nurse – Long-Term Care
Location: Phoenix, AZ
Type: Contract

Shift & Schedule: Night Shift | 6:30 PM – 6:00 AM | 12-Hour Shifts | Approximately 40 Hours Per Week

Pay Rate: $40 - $42/hour

Position Overview:

Navitas Healthcare, LLC is seeking a Licensed Practical Nurse – Long-Term Care for an exciting Local job in Phoenix, AZ.

Responsibilities

  • Administer medications and treatments as prescribed and within the scope of LPN practice.

  • Perform nursing procedures including wound care, catheter care, tracheostomy care, and enteral feeding care.

  • Monitor residents and evaluate the effectiveness of medications and treatments.

  • Observe, document, and report changes in resident condition to the healthcare team.

  • Participate in shift reports and communicate resident care needs effectively.

  • Complete admission assessments for newly admitted residents.

  • Assist with MDS assessments, quarterly reviews, and care plan development.

  • Maintain accurate clinical documentation, treatment records, and resident progress notes.

  • Communicate effectively with residents, families, physicians, and interdisciplinary team members.

  • Ensure adequate supplies and medications are available for resident care.

  • Participate in Quality Assurance and Performance Improvement (QAPI) initiatives.

  • Follow all federal, state, facility, and nursing practice regulations.

Required Qualifications

  • Active Arizona Licensed Practical Nurse License.

  • Current and unrestricted nursing license in good standing.

  • Knowledge of geriatric and long-term care nursing practices.

  • Ability to assess resident conditions and respond appropriately to medical concerns and emergencies.

  • Strong clinical documentation and medication administration skills.

  • Excellent verbal and written communication skills.

  • Ability to work effectively within an interdisciplinary healthcare team.

  • Ability to maintain professionalism and confidentiality in all resident interactions.

Preferred Qualifications

  • Long-Term Care experience.

  • Skilled Nursing Facility experience.

  • Experience with MDS documentation and care planning.

  • Experience working with geriatric populations.
