Job Title: Clinical Social Worker (LICSW)

Location: Hyannis, MA
Duration: 13 Weeks
Schedule: 40 Hours per Week | 8:00 AM – 4:00 PM | Rotating Days, Rotating Weekends, Occasional Holidays

Compensation

Travel Pay: $65–$70/hr ($2,437–$2,625/week) (Based on Experience)
Local Pay: $60–$63/hr ($2,250–$2,362/week) (Based on Experience)

Position Overview

We are seeking an experienced Licensed Independent Clinical Social Worker (LICSW) to provide comprehensive psychosocial services to patients and families within an acute care healthcare setting. The Social Worker will assess patient needs, facilitate discharge planning, coordinate community resources, provide crisis intervention, and support patients and families coping with illness, hospitalization, and life transitions.

Responsibilities

Psychosocial Assessment & Patient Support

  • Conduct comprehensive psychosocial assessments of patients and family members.
  • Evaluate home environments, family dynamics, mental health history, substance use history, cultural considerations, and socioeconomic factors.
  • Complete risk assessments and interventions for vulnerable populations, including children, elderly individuals, disabled patients, and victims of abuse or neglect.
  • Provide emotional support, crisis intervention, counseling, and advocacy services.
  • Assist patients and families in understanding treatment recommendations and healthcare plans.

Care Coordination & Discharge Planning

  • Collaborate with interdisciplinary healthcare teams to develop individualized discharge plans.
  • Coordinate safe discharge planning and continuity of care.
  • Evaluate ongoing medical and psychosocial needs and arrange appropriate community services.
  • Assist patients and families with placement decisions, long-term care options, and post-acute care services.
  • Coordinate access to medical equipment, home services, and community-based resources.

Community Resource Management

  • Serve as a liaison between patients, families, healthcare providers, and community agencies.
  • Facilitate referrals to appropriate behavioral health, substance abuse, social service, and community support programs.
  • Evaluate the effectiveness of community services and recommend alternative resources when needed.
  • Assist patients in navigating insurance benefits, third-party payers, and support programs.

Documentation & Compliance

  • Maintain accurate and timely documentation within electronic health record systems.
  • Ensure compliance with professional social work standards, organizational policies, and regulatory requirements.
  • Participate in quality improvement initiatives and departmental meetings.
  • Maintain patient confidentiality and adhere to the Social Work Code of Ethics.

Team Collaboration

  • Work collaboratively with physicians, nurses, case managers, behavioral health professionals, and community agencies.
  • Participate in multidisciplinary care planning and patient rounds.
  • Support departmental initiatives and committee participation as assigned.

Required Qualifications

Education

  • Master’s Degree in Social Work (MSW) from an accredited program.

Licensure & Certifications

  • Current Massachusetts Licensed Independent Clinical Social Worker (LICSW) License.
  • CPI (Crisis Prevention Intervention) and/or MAB certification required within one month of hire and maintained annually.

Experience

  • Acute care hospital social work experience preferred.
  • Experience with discharge planning, crisis intervention, behavioral health assessments, and care coordination preferred.

  • Ability to work independently and effectively in a fast-paced healthcare environment.

  • Ability to read, write, and communicate effectively in English.
