Job Title: LPN/LVN – Long Term Care (LTC)
Location: New Lisbon, WI
Duration: 13 Weeks
Shift: Full-Time PM Shift (2:00 PM – 10:00 PM), Every Other Weekend

Position Summary: We are seeking a dedicated Licensed Practical Nurse (LPN/LVN) to provide quality nursing care in a Long-Term Care setting. The role involves administering medications, monitoring resident health, and ensuring effective communication with healthcare teams, residents, and families.

Key Responsibilities:

  • Administer medications following the “Five Rights” of medication administration
  • Monitor residents and report any changes in condition to the Charge Nurse
  • Communicate significant changes in resident status to physicians and family members
  • Report any concerns related to abuse, neglect, or misappropriation to leadership
  • Assist Certified Nursing Assistants (CNAs) with resident care and call lights as needed
  • Maintain accurate documentation and ensure clear communication among care teams
  • Perform additional duties as assigned

Required Qualifications:

  • Active Wisconsin Licensed Practical Nurse (LPN) license
  • Current BLS/CPR certification
  • Long-Term Care experience preferred
  • Strong attention to detail and accuracy
  • Excellent communication and teamwork skills
